Freedom of Information Act
Requests for information under the Freedom of Information Act should be made in writing and directed to:
Allen Albus, FOIA Officer, School District 115, 300 S. Waukegan Rd., Lake Forest, IL 60045.
Any person requesting records of Lake Forest High School District 115 may make such a request in person, orally or in writing, at the board of education office at 300 S Waukegan Rd., Lake Forest, Illinois. Such request should be made to the district’s Freedom of Information Officers, Mr. Allen Albus, Deputy Superintendent for Finance and Operations or Ms. Jennifer
Hermes, Assistant Superintendent for Business Services at such address; and if he is not present, such request may be made to Ms. Pam Burke, Board Clerk.
Alternatively, any person may mail a written request to either Mr. Albus or Ms. Hermes specifying in particular the records requested to be disclosed and copied. All written requests should be addressed to the board of education office at the above address. If you desire that any records be certified, you must indicate that in your request and specify which records must be certified. Records may be available in paper copies, pdf's, or, depending on the original format, various software programs.

The first 50 pages of any request are free, pages in excess of 50 maybe charged at $.04 per page.
Most requests will be responded to within 5 business days of receipt.
